: A pancreatic pseudocyst (PC) is a capsulated collection of homogeneous fluid that is a common complication of pancreatitis, resulting from damage to the pancreatic ducts caused by gallstones or excessive alcohol consumption. Symptoms are often non-specific, but symptomatic cysts that do not resolve spontaneously require operative treatment. It consists of percutaneous drainage (PD), ultrasound endoscopy (EUS) drainage and surgical drainage. When surgery is the only choice possible, the most common surgical procedures are cystogastrostomy (CG) and the Roux-en-Y-type cystojejunostomy (RCGJ). This video shows the case of a 49-year-old man with 2 different and not linked PC. PD and EUS drainage were not possible, so robotic-assisted RCGJ and a cystocystostomy were performed. Operational time was 280 min with an estimated blood loss of 220 ml. The patient was discharged on postoperative day 7. This video clearly shows all the necessary steps to perform this surgical procedure.
