Pseudoaneurysms are rare but life-threatening complications of myocardial infarction (MI) that require a prompt diagnosis. However, this is challenging in clinical practice, especially when clinically silent. We present the case of a 63-year-old man with 2 pseudoaneurysms identified by means of cardiac magnetic resonance (CMR) after a single subacute MI. To avoid the risk of an impending rupture, the patient underwent left ventricular assist device implantation and subsequently heart transplantation. CMR imaging in the acute phase allowed early detection of the pseudoaneurysms and avoided fatal complications. The histologic examination of the explanted heart perfectly matched CMR findings.
